Terms and Conditions
These terms apply to NDIS supports delivered by Cyrus Ability Care (ABN 49 676 216 363), a registered NDIS provider. They should be read alongside each participant’s service agreement.
Our Services
We deliver NDIS supports across the registration groups we hold, including personal care, high intensity daily personal activities, community nursing, household tasks, community access, life skills, travel and transport, and employment support. Supports are delivered in line with the NDIS Practice Standards and each participant’s plan and service agreement.
Service Agreements
Before support begins, we agree the supports, schedule, and any specific arrangements with the participant or their representative. The service agreement sets out what we will deliver and what the participant can expect.
Payment
We charge in line with the NDIS Pricing Arrangements and Price Limits. How supports are paid depends on the participant’s plan:
NDIA-managed: we claim payment directly from the NDIS
Plan-managed: we invoice the participant’s plan manager
Self-managed: we invoice the participant, who claims from the NDIS
Cancellations
We understand plans change. If a participant needs to cancel or reschedule a support, we ask for as much notice as possible.
A short notice cancellation is a cancellation made with less than seven clear days’ notice, or where the participant is not present at the agreed time and place. In line with the NDIS Pricing Arrangements, short notice cancellations may be charged. We apply these rules fairly and discuss any concerns with the participant.
Our Responsibilities
We will deliver supports safely and reliably, keep the same workers where possible, communicate clearly, protect personal information, and meet our obligations under the NDIS Practice Standards.
Participant Responsibilities
We ask participants to provide a safe environment for workers, give notice of changes or cancellations where possible, treat workers with respect, and let us know if their needs or plan change.
Ending a Service Agreement
Either party can end a service agreement in line with the notice period set out in the agreement. We will support a smooth handover to another provider where needed.
